Players get to name their teams, their team colors, logos and have access to several other vanity items, including picking their favorite team and state – these last two having a special impact on the gameplay experience. Fantasy teams can be assembled by picking from over 900 active major league players.

Filling the shoes of a baseball team manager, players have the ability to scout, trade and train the members of their squads and then pitch them against other player-made teams. Like many other similar games, MLB Manager Online allows some fine-tuning of squad behavior, such as deciding the pitching order, batting order or positions on the pitch. Team members' attributes can be easily enhanced with the help of training cards which everyone can purchase in the form of packs. They offer bonuses to one of the six stats that define a player’s performance: Stamina, Speed, Power, Break, Control and Clutch for pitchers and Contact, Power, Run, Throw, Field and Clutch for position players.

While holding a lot of potential, the game lacks clarity for some of its features. One of them is the hometown component which seems to develop at the same rate your team does, yet the way it influences gameplay is fuzzy. Others include interface elements that lack tooltips, manager career choices that impact the player's experience in unexpected and unintuitive ways.

MLB Manager Online is based on micro-transaction system which allows the purchase of SEGA Coins which in turn enhance the gameplay experience by offering otherwise unavailable game options. The monetization engine is called UltimatePay and is developed by PlaySpan.
